The OGE Form 201 is used to request copies of the public financial disclosure reports and other specified records of certain executive branch officials. These records are available under the Ethics in Government Act of 1978 ("the Act") and amendments thereto (5 U.S.C. app. 103, 105 and 402), the STOCK Act, and OGE's implementing regulations. The Act imposes restrictions on the use of the records. They cannot be used:
- for any unlawful purpose
- for any commercial purpose, other than by news and communications media for dissemination to the general public
- for determining or establishing the credit rating of any individual
- for use, directly or indirectly, in the solicitation of money for any political, charitable, or other purpose.
Available from OGE
Copies of the certified public financial disclosure reports (SF 278/OGE Form 278/OGE Form 278-T) and, as appropriate, Certificates of Divestiture for individuals who have been nominated by the President to executive branch positions requiring Senate confirmation may be obtained by completing the automated OGE Form 201, "Request to Inspect or Receive Copies of Executive Branch Personnel Public Financial Disclosure Reports or Other Covered Records." In addition, OGE reviews the public financial disclosure reports of individuals who have declared their candidacy for the Office of President of the United States. Those certified reports may also be obtained by completing the automated OGE Form 201.
The public financial disclosure reports and certificates of divestiture of designated agency ethics officials and certain other officials are available from OGE by filing the paper version of the OGE Form 201 with OGE.
Other records, referred to as “covered records,” of Presidential appointees subject to Senate confirmation, designated agency ethics officials, and certain other officials are also available from OGE by filing the paper version of the OGE Form 201. "Other covered records" include:
- certain Ethics Act qualified blind trust and qualified diversified trust documents
- other OGE Form 201s

Available from Executive Branch Departments and Agencies
The public financial disclosure reports and “covered records” of all other public filers are available from the employing departments and agencies. Submit the OGE Form 201 to the Designated Agency Ethics Official at the agency employing the filer in which you are interested.
